DentaQuest Value-Added Services give kids rewards for good dental care.

The chart below explains more. The kits are easy to get. All you have to do is:

• When you go to your dentist, bring the value-added services form from your member handbook.
• Fill out the form.
• Have your dentist fill out and sign the bottom of the form.
• Mail the signed form to DentaQuest in the envelope attached to the bottom of the form.

Ages 12 months to 3 years old.

Needed Dental Treatment: Dental check-up
Healthy Beginnings Toddler Pack with an infant toothbrush, a finger brush, and teething ring. Limit 1 per member.

Ages 3 years to 5 years old.

Needed Dental Treatment: Topical fluoride treatment
Big Kids Backpack with a toothbrush, toothpaste, brushing chart, and stickers. Limit 1 per member.

Ages 5 years to 14 years old.

Needed Dental Treatment: Sealants
Sports Backpack with a mouth guard or water bottle, toothbrush, and toothpaste. Limit 1 per member.

Ages 14 years to 20 years old.

Needed Dental Treatment: 2 teeth cleanings in a year
Sports Backpack with a mouth guard or water bottle, toothbrush, and toothpaste. Limit 1 per member.
